Immunization Requirements

Parents must provide physician records or an official record produced by a state or local agency documenting a completed series of immunizations upon entry to Conroe ISD schools for their child.  Each received vaccine must list the month/day/year to be considered a valid immunization.  If a student is currently receiving an immunization series, he/she must receive the series as rapidly as medically feasible and continue to provide updated physician records to the school clinic.

Immunization Requirements for 5th and 6th Graders

5 or 4 doses
DPT/DTaP/DT/Td
(with 1 dose on or after the 4th birthday)  For students age 7 and older 3 doses meets requirements if one dose received after 4th birthday.
4 doses
Polio
(only 3 doses if one dose was given after the 4th birthday, 6 months between last 2 doses)
2 doses
MMR
(received on or after the 1st birthday, given same time as Varicella or 1 month later)  See note.
3 doses
Hepatitis B
For students ages 11-15 years, 2 doses meet the requirement if adult hepatitis B vaccine (Recombivax) was recieved.  Dosage and type of vaccine must be clearly documented.  Two (2) 10 mcg/1.0 ml doses of Recombivax.  If Recombivax is not the vaccine received, a 3-dose series is required.
2 doses
Hepatitis A
The 1st dose of hepatitis A must be received on or after the 1st birthday.
2 doses
Varicella*
(unless history of disease, must have 3 months between doses)  Only 1 dose required.
*Varicella Immunization is not required if the student has had the disease (chicken pox). Parents, guardians, school nurses and physicians may documen t history of illness.
